Simone Allen, a Houston mother, is suing Cullen Pharmacy for a prescription error that she claims took the life of her six-year-old daughter, Jadalyn, in April of 2012. Jadalyn suffered from sickle cell anemia and had been taking a liquid painkiller to deal with discomfort.
As pain increased, the girl was prescribed a low dose of morphine, dispensed at 20 milligrams per 10 milliliters. Cullen Pharmacy gave Allen a dose of 20 milligrams per 1 milliliter, 10-times higher than Jadalyn’s prescription called for, which turned out to be too much for the young child’s body.
